In order to be accepted for a Direct Family Care Membership you need to have an active Debit/Credit Card that you are authorized to use for our services.

There will be no charges on your card until we accept you into the Direct Family Care program.  We will then contact you to verify that you have authorized your membership, and we will discuss with you what day your card will be automatically billed monthy.

There is a minimum of a 3 month commitment to Direct Family Care. If you are accepted for a Direct Family Care Membership you agree to having your Debit/Credit Card/Bank Account charged a for the membership fee that you agree to for a minimum of 3 months. If at any time your card doesn’t work, you will be mailed a bill. (See FAQ on lapse of membership)

If you have dependents, you must complete the application process for each legal dependent.

Please be aware that you are going to be asked multiple medical questions during your application process.  Please make sure you be as detailed as possible in order to appropriately process your enrollment.

(NOTE) If you have an employer that has an agreement with us, you must still sign up with a Credit/Debit Card.  We will not charge your card for your portion, unless it is a part of the agreement that you have made with your employer.
